Please note that the two forms here: the Institutional Certification and Study Sharing Plan and Submission Information. These should be submitted to the DCC ( pritham.m.ram@uth.tmc.edu ) for each project, after approval by the respective Program Officer. The DCC will send the completed forms to NIH on a monthly basis for uploading all the information into dbGaP system. Please reach us for any queries concerning these.
